Western Victoria Primary Health Network Together with our partners and communities, Western Victoria Primary Health Network (PHN) identifies priority health care needs, improves access through government funding, and co-designs localised solutions to improve health care systems across western Victoria.
Western Victoria PHN is one of 31 Primary Health Networks across Australia and one of six in Victoria that was established on 1 July 2015. We serve a population of approximately 600,000 people with a land area of 79,843sq kms. Each part of the region has established Regional Integrated Councils to provide the organisation with insights into key regional healthcare needs. These Councils comprise healthcare professionals and community representatives.
We are responsible for delivering on the following two objectives set by the Australian Government:
- increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes and
- improving coordination of care to ensure patients receive the right care, in the right place, at the right time.
